Team Registration

BelovED Athletics has moved to a mostly electronic registration and submission of forms for all athletic programs. Students that intend to participate in athletics are asked to complete the following:



Step 1. Register Online via the registration form emailed home.


In the email find the athletic team/program your child is interested in and completely fill out the form for that team. 


Step 2. Download and fill our appropriate Athletic Physical Form(s).


The NJ Athletic Pre-Participation Physical Form is required for any student that is registering for interscholastic athletic participation.  


The first page, The History Form, is required for all and is completed by a parent/guardian. 


The following pages of the form must be completed by a NJ licensed physician, and MUST have the physician's original signature and office stamp on the Clearance Page or it will be rejected. 


These forms must be downloaded, printed, completed by a parent/guardian and a physician and then returned to the school office.


If you have submitted a sports physical form to the athletics office within 365 days of the start of the fall season (August 10th, 2020) then you do not need to submit a new one but you do need to complete the Health History Update Form.


The Health History Update Form is required for any student that has an Athletic Preparticipation Athletic Form on file from a previous season with a date of examination that is within 365 days of the first day of tryouts/practice (After August 10, 2019). 


 This form must be downloaded, printed, completed by a parent/guardian and then returned to the school office.


Step 3. Clearance email


Updates to the status of your athletic registration will be emailed to you after your submission and review. Please check your emails for any communications regarding issues with your submission. The earlier you submit your registration and forms the sooner the likelihood you will receive medical clearance. 

Information on Getting an Examination


We realize that if you have not had an annual examination, depending on your private physician’s office hours, it may be difficult to get an appointment. There may be changes coming from the NJDOE to this regulation but until we hear otherwise, we must follow the state guidance on this requirement. At this time, physical exams are still required to be completed within 365 days of the first tryout/practice. If you need an examination in order to get the forms completed there are a few options:


Contact your private physician to secure an appointment for an athletic physical. They can tell you what they are doing regarding examinations.


Contact a local urgent care facility to schedule an athletic physical. Bring a blank form with you for completion. CityMD in Journal Square and downtown and CVS in Hoboken all do sports physicals.
